This page provides access to the forms used by the members of AMTA for the normal conduct of business. The user can view any form by clicking on the form name, to display a copy of the form. To print, click on "File" at top left of the display and scroll down to print.

AMTA Directory Information Form The form provides a medium to submit the information used in the published directory and Teacher Directory on www.amtadir.org website. The form can be downloaded from the pdf file, printed, completed and mailed to the Sec/Treasurer and to the Webmaster at the Maestro Music address listed below. Listing is for AMTA members, consistent with membership in MTNA and PMTNM.

Interractive Directory Change or Update Form Members can quickly update their listing in the on-line directory. The form can be completed on the computer screen and transmitted directly to the Webmaster, or the completed form can be printed out for your records, with a copy mailed to the Webmaster at the Maestro Music address listed below.

AMTA Reimbursement Form - Use for expenses incurred by members in the conduct of official AMTA activities. All expenses must be approved by AMTA board representative and be documented with receipts.

Student Recitals: Down load 2008/09 Student Recital form and Submit to co-chair for a student wishing to participate in Student Recital Series. Recitals are currently scheduled for September 28, 2008, November 9, March 1, 2009, and April 5. All recitals are held at Charles Pianos starting at 2:00 pm.   A completed copy of the form must be in the hands of Jody Nelson, co-chair, at least 10 days in advance of the recital in which the student will participate.
